The Return to Work form feature in the EH Work app allows you to complete and submit your return to work documentation directly from your mobile device. This convenient mobile solution means you can complete your form anytime, anywhere - whether you're commuting back to work or at home. The form captures the same information as the web version, including talking points, comments, attachments, and your declaration, all optimised for mobile use.
Getting started
A Return to Work form becomes available when your approved time off is ending. You'll receive an in-app notification that says "A Return to Work form is now available (time off ending: DD MMM YYYY)" when the form is ready for completion.
The form will show one of three statuses:
- Not Started - The form is available but hasn't been opened yet
- In Progress - You've started answering questions or adding attachments
- Completed - Both you and your manager have submitted your declarations
- Open the EH Work app on your mobile device.
- Tap the notification about your Return to Work form (if you received one), or navigate manually.
- To navigate manually:
- Tap Work at the bottom of your screen
- Tap Leave in the top navigation
- Find your relevant time off request in the Recent history section
- Tap on the time off request to open Leave Request Details
- On the Leave Request Details page, you'll see the Return to Work form section with its current status.
- Tap Return to Work form to open the form.
Completing your form
- In the Talking points section, you'll see a list of questions you need to answer.
- Tap on any question to open it (for example, "What was the reason for the employee's absence?").
- You'll see a text box where you can Write a response.
- Type your answer in the text field.
- Tap Save to save your response.
- Return to the main form by tapping the back arrow.
- Repeat this process for each question you want to answer.
- You'll see the number of answers saved next to each question (e.g., "1 answer").
Helpful Hint
Use the Reload button at the top of the form to refresh and see any new answers that have been saved by your manager or other parties.
- Scroll down to the Comment section.
- Tap Add comments.
- Write any additional comments you want to include with your form.
- Tap Save to save your comments.
- Scroll down and tap Add attachment.
- Choose from the following options:
- Take photo... - Use your phone's camera to take a new photo
- Select photo... - Choose an existing photo from your device
- Select file... - Choose a file from your device
- The form accepts jpeg, png, and pdf file types with a maximum file size of 10MB.
- Once uploaded, you'll see the file name displayed in the attachments section.
- To remove an attachment, tap the trash/delete icon next to the file name.
- Once you've answered at least one question or added an attachment, scroll down to the bottom of the form.
- You'll see a declaration checkbox with the text: "I, [Your Name], agree to the accuracy of the information contained in this form and I confirm that I am able to return to work."
- Tap the checkbox to tick it and agree to the declaration.
- Tap Submit declaration to submit your form.
- You'll see a confirmation message: "Awesome! Declaration submitted"
- The message will also remind you that "You can come back to view this Return to Work form at any time within your Time Off Requests."
- Tap Got it to acknowledge the confirmation.
Important
Once you submit your declaration, the form will become locked and you won't be able to make further changes. Make sure all your information is accurate before submitting.
Form status updates
Your Return to Work form status will update automatically based on your actions:
- Not Started - The form is available but no answers or attachments have been saved yet
- In Progress - You've saved at least one answer or added an attachment, but haven't submitted your declaration yet
- Completed - Both you and your manager have submitted your declarations
You can always return to view your completed form by going to Work Leave finding your time off request Leave Request Details Return to Work form.
Troubleshooting
If you can't find your Return to Work form:
- Make sure you're looking in the correct time off request in your Recent history
- Check that your time off has been approved and is ending soon
- Try refreshing the app by pulling down on the screen
- Make sure you have the latest version of the EH Work app installed
- Contact your HR team if the form should be available but isn't showing
If you're having trouble uploading attachments:
- Check that your file is in jpeg, png, or pdf format
- Ensure your file is smaller than 10MB
- Make sure you have a stable internet connection
- Try closing and reopening the form
- If the problem persists, try completing the form on the web version
